Home Health Care  (License No. HHA 21196096)

UCH Home Health was created to provide an alternative to nursing home or extended hospital stays and to provide continuous, quality care from the hospital to the home. Our professional staff of nurses work with the patient and physician to provide home health services that meet the specific medical and personal needs of the patient in a compassionate and caring manner.

Patients may be eligible for home health services even if they have not been hospitalized.  Careful monitoring and teaching can prevent hospitalization and re-hospitalization in many instances.  Patients need not be home-bound for non-Medicare covered services.

Through UCH Home Health, a personalized care plan will be developed by the physician and supervised by a registered nurse.  Other members of our skilled care team include: physical therapists, occupational therapists, speech therapists, medical social workers and home health aides.  Patients and family will be taught how to care for the patient when the nurse or caregiver is no longer needed in the home.  Because a registered nurse is on-call 24-hours a day, seven days a week, UCH Home Health is just a phone call away at (813) 558-4910.
